Quick Summary

The service delivers a flexible and person-centred Emergency Department (ED) Navigator model that works towards removing the barriers to accessing support in the community, improving health outcomes for those presenting in ED with violence-related injuries.

The provision is available to residents aged 11 – 35 years, in the Humber Region presenting in ED with a violence related injury due to physical interpersonal violence, including family, partner or community violence, and sexual assault.


Navigators work with individuals to design a tailored support plan to enable the young person to be connected to appropriate services following a needs assessment. They also follow up with the young person and their family following discharge. The ED Navigator service provides trauma-informed care, taking into account Adverse Childhood Experiences (ACEs) and multiple-trauma.


The service works with key internal and external stakeholders to develop and embed a sustainable best practice response to improved identification of violence related injuries in ED and access to support in the community. It helps by co-ordinating a multi-agency response that aims to reduce and manage the risk identified.

The level and frequency of support provided are tailored to each client’s individual needs, risks, and the circumstances surrounding their attendance in ED. Engagement is therefore flexible and responsive rather than fixed. 

Throughout the intervention, flexibility remains key. If a client’s circumstances change or risks increase, the level of support can be adjusted accordingly to ensure their safety and continued engagement.
